A Registration Form must be completed each season. Registration will not be finalized without this form.
Annual Information Form
To register for any SSSRA programs, an Annual Information Form must be updated and filed with SSSRA on a yearly basis. Registration will not be complete unless we have an updated form on file.
This form includes more detailed questions in an effort for our staff to better serve you or your child.
Medication Dispensing Form
A Medication Dispensing Form is required if medication will be dispensed during programs or special events.
